The National Transportation Safety Board has released its preliminary report into the fatal crash of a small airplane off the coast of San Mateo County last spring, revealing that the crash was caused by issues with the auxiliary fuel systems needed for the transpacific flight. The crash resulted in the tragic deaths of both the pilot and the sole passenger on May 20, 2023, at approximately 33 miles off the California coast at 2 p.m.
The plane, registered to the Bank of Utah, was en route to Daniel K Inouye International Airport in Honolulu and was equipped with a ferry tank system containing 540 gallons of fuel necessary for the trip to Hawaii. However, the airplane’s amphibious floats had been removed prior to the flight. Satellite flight tracking data indicated that about four hours into the flight, the crew reported a problem transferring fuel from the ferry tanks and intended to return. As the plane was about 356 miles from the California coast, it turned northeast. The flight crew later reported a loss of fuel pressure and engine failure, and the plane ultimately crashed into the ocean.
